Abstract

The current disjointed “process” of drug prescribing, dispensing, administration, and consumption frequently results in drug therapy outcomes that are less effective, appropriate, safe, and economical than is either possible or desirable. The shortcomings of the current system do not stem so much from the fact that several professionals and the patient are involved, but rather from the lack of effective collaboration, communication, and coordination of pharmaceutical care delivery among them. This commentary briefly reviews the current medication prescribing and use process within its historical context, and offers a vision for implementation of a more collaborative and interdisciplinary system of care which can improve the outcomes and cost-effectiveness of drug therapy.
